1. Operation without a permit or interim status 40 CFR Part 270, 262.34.
2. Failure to submit a copy of its revised contingency plan to all local police departments, fire departments, hospitals, and State and local emergency response teams that may be called upon to provide emergency services at the Facility.
3. Failing to keep a container, holding hazardous waste at the Facility closed at all times during sotrage, except when it was necessary toadd or remove waste.
