This case stems from information gathered during and after an EPCRA 313 mult-media inspection was conducted at the facility from December 1 -3, 2004. The facility is engaged in the production of expanded shale aggregate. The facility failed to submit Form R reports for a number of compounds otherwise used at its facility from 2000-2003.
