The facility failed to prepare and implement written operation procedures that provide cleaf instructions or steps for safetly conducting activities associated with the covered process, including written procedures for normal operations and emergency shut-down of the 20 pound cylinder filling station and mechanical integrity procedures, as required under CAA Section 112(r)(7) and 40 CFR Part 68.69(a) and 68.73(b) and (d)(7).
