Respondent operates an injection well in the form of a large capacity septic system at the facility. On April 24, 2007, EPA sent a Permit/Closure Plan and Request for Information (SDWA-UIC-IR-07-099) to the Respondent. Respondent has not submitted a complete permit application or ceased injection.
