This Administrative Order is issued to Glen Mikesell, owner of Glen's Mountain View Cafe, for exceeding the MCL for nitrate, failure to collect and analyze confirmation samples after an exceedance of the MCL for nitrate, exceeding the MCL for total coliform bacteria, failure to monitor for bacteriological quality, failure to provide public notice, and failure to notify the State and EPA of the violations.
