Respondent operates oil & gas facilities in the Outer Continental Shelf of the Gulf of Mexico, is a permittee; subject to regulations of the Clean Water Act and National Pollutant Discharge Elimination System. Respondent reported effluent violations, did not have NPDES general permit coverage, failed to perform sample toxicity testing, and failed to follow reporting instructions.
