This facility failed to submit the Material Safety Data Sheets for calcium carbonate, barium sulfate and talc to the State Emergency Response Commission, the Local Emergency Planning Committee, and the local fire department with jurisdiction over the facility. Additionally, the facility failed to submit the Emergency and Hazardous Chemical Inventory forms to the SERC, the LEPC, and the local fire department for the 1998-2002 calendar years, by the March 1 deadline.
