Facility was inspected and issued NOVs. Facility inadequately responded to NOVs and failed to correct all violations until 4 to 6 months after NOVs was issued. These violations included failure to make arrangements with local officials, inadequate training and inadequate contingency plan. An Opportunity to confer meeting was held prior to filing of complaint. Respondent agreed to pay penalty of $14,567.00.
