Respondent violated Part II.C.2 of the permit by failing to prominenlty post a copy of the Notice of Intent (NOI) at the construction site for public viewing. Respondent violated Part IV.D.1.e of the Permit by failing to indicate the location, latitude and longitude of the storm water water outalls on the Storm Water Pollution Prevention Plan (SWPPP) site map. Respondent violated Part IV.E of the Permit by failing to include contractor's and subcontrators' certification statements in the SWPPP. Respondent violated Part IV.D.2.a(1) of the Permit by failing to implement stabilization practices in areas where construction activity temporarily or permanently ceases. Respondent violated Part IV.D.2.a(2) of the Permit by failing to adequately install and maintain structural practices to limit runoff and the discharge of pollutants from exposed areas of the site to the degree attainable. Respondent violated Part IV.D.2.c(2) of the Permit by failing to minimize off-site vehicle tracking. Responde violated Part VI.D of the Permit by failing to prevent discharges which have a reasonable likelihood of having an adverse impact on eithr human health or the environment. Respondent violated Part VI.N of the Permit by failing to properly operate and maintain all facilities and systems of treatment and control whcih are installed to achieve compliance with the Permit and the SWP
