Respondent violated Part II.C.2 of the Permit by failing to prominently post a copy of the Notice of Intent (NOI) at the construction stie for public viewing. Respondent violated Part IV.D.1.e of the Permit by failing to indicate the location, latitude and longitude ofthe storm water outfalls on the Storm Water Pollution Prevention Plan (SWPPP) site map. Respondent violated Part IV.E of the Permit by failing to include contractors' and subcontrators' certification statements in the SWPPP. Respondent violated Part IV.D.2.a(1) of the Permit by failing to implement stabilization practices in areas where constructions activity temporarily or permanently ceases. Respondent violated Part IV.D.2a(2) of the Permit by failing to adequately install and maintain structural practices to limit runoff and the discharge of pollutants from exposed areas of the site to the degree attainable. Respondent violated Part VI.D of the Permit by failing to prevent discharges which have a reasonable likelihood of having an adverse impact on either human health or the environment. Respondent viiolated PART VI.N. of the Permit by failing to properly operate and maintain all facilities and systems of treatment and control which are installed to achieve compliance with the Permit and the SWPPP
