BUR violated various waste manifest requirements when it shipped a 5,000-gallon load of PCB-contaminated waste to non-PCB disposal facility in April 2008. Specifically, BUR failed to: identify the waste as PCB-contaminated; list the weight of the waste in kilograms; and indicate the earliest date of removal of the PCBs from service.
