Administrative Order (AO)issued on February 11, 2009 for failure to sample for TTHM and HAA5 during May, August and November of 2008 according to system's standard monitoring plan for Stage 2 Disinfectant Byproducts Rule (Stage 2 DBPR). The AO requires the system to conduct standard monitoring for TTHM and HAA5 at 4 standard monitoring sites according to its revised standard monitoring plan. System met the requirements of the AO by monitoring for total trihalomethanes (TTHM) and haloacetic acids (HAA5) according to its revised Stage 2 Disinfectants and Disinfection Byproducts Rule (Stage 2 DBPR) standard monitoring plan which was approved by EPA on February 12, 2009. System submitted an Initial Distribution System Report on December 15, 2009, and EPA issued Sheffield Lake City PWS a Notification of IDSE Report Completeness on December 15, 2009. EPA issued system a letter terminating the AO on 1/20/2010.
