* Inadequate or no prediction of equipment failure which could dresult in discharges.
* Plan does not idscuss and facility does not implement appropriate containment/diversionary structures/equipment-(inlcuding truck transfer areas).
* Inspections and tests required by 40 CFR Part 112 are nto in accordance with written procedures developed for the facility.
* Are not signed by appropriate supervisor or inspector.
* Are not kept with the plan.
* Are not maintained for three years.
* Spill prevention briefings are not scheduled and conducted periodically.
