SWI facility has not been in compliance w/the requirements of the HMIWI Rule (40 CFR Sec 62.14400(b)) from at least 10/1/02 thru present including: (a) Waste Management Plan (40 CFR Sec 62.14451), (2) performance testing and monitoring requirements (40 CFR Secs 62.14451, 62, 14453, 62.14454, (3) reporting and recordkeeping requirements (40 CFR Secs 61.14460 62.14463, 62.14464) and compliance schedule (40 CFR Sec 62.14470).
