Administrative Order (AO) for failure to collect a Cryptosporidium spike sample for its December 2008, and January, February, and March 2009 Cryptosporidium field samples which invalidated these samples. The AO requires the system to revise and submit an approvable source water monitoring plan, and collect a Cryptosporidium matrix spike sample and Cryptosporidium, E. coli, and turbidity field samples in December 2009 through March 2010, and submit a bin classification to Region 5 U.S. EPA. The AO also requires the system to notify its customers of the violations.
