This Administrative Order is issued to Eagle Butte Public Water System for failure to submit a Stage 2 Disinfection Byproducts standard monitoring plan by the required timeframe, failure to comply with the analytical and monitoring requirements for chloramines, failure to notify the public of the violations and failure to report the violations to the State and EPA.
