This facility disclosed the following: 1) the Henderson District constructed a stationary source with emissions of PM and PM-10 that exceed regulatory thresholds requiring an Air Pollution Emission Notice and a construction permit; 2) the Henderson District did not submit an APEN and construction permit application; and 3) the Henderson District began operation of the facility prior to receiving an initial approval emission permit.
