The CAFO sets forth the following violations applicable to both RespondentÂs Pulaski and Baylis Facilities in Baltimore, MD:
1. Operating a hazardous waste treatment, storage or disposal facility without a permit or interim status;
2. Failure to keep hazardous waste containers closed during storage;
3. Failure to maintain an inspection log of weekly hazardous waste inspections;
4. Failure to provide hazardous waste training;
5. Failure to prepare and maintain hazardous waste training records;
6. Failure to make hazardous waste determinations; and
7. Offering hazardous waste to a TSDF without an EPA Identification Number.
In addition, the CAFO sets forth the following additional violations for the Baylis Facility:
8. Failure to prepare and/or maintain LDR forms;
9. Failure to prepare hazardous waste manifests; and
10. Transporting hazardous waste without an EPA Identification Number.
