This Administrative Order is issued to the Town of Fairview for failure to provide notification of lead tap exceedance to the occupants of the residence where the sample was taken, failure to conduct public education for lead, failure to monitor the water for lead and copper, failure to develop and submit an optimal corrosion control treatment recommendation and a source water treatment recommendation, failure to monitor for water quality parameters, failure to monitor for volatile organic chemicals, failure to notify the public of the violations, and failure to report the violations to the State and EPA.
