A PCB inspection conducted on July 17, 2008 revealed that Ox failed to prepare quarterly inspection documents and annual documents for two in-use PCB transformers at their Halltown, WV facility. Additionally, the access gates to the transformer substations were not marked with a PCB label. They signed a SCAFO on June 7, 2010 agreeing to settle the violations for a penalty of $30,600.
