This enforcement action was filed in response to violations to CAA 112(r)(7) which were discovered during a Risk Management Plan (RMP) inspection conducted on 08/28/2008. Violations were found in the following areas: written process safety information, process hazard analysis, certification that operating procedures were current and accurate, training and training records, inspection and testing of facility equipment and documentation of audit deficiencies and corrections.
