This Field Citation was issued due to the facility's failure to: 1) continue leak detection in temporary closure; 2) continue cathodic protection in temporary closure; and 3) notify EPA/NNEPA of a change-in-service. The violations were addressed by removing the UST and submitting a UST notification form.
