This enforcement action was filed in response to Risk Management Plan violations discovered during a CAA 112(r)(7) compliance inspection which was conducted on 6/8/2010. Violations include: 1.) Failure to update and revalidate the process hazard analysis in a timely manner; 2.) Failure to annually certify that the operating procedures are current and accurate; 3.) Failure to implement their own maintenance procedures; 4.) Failure to implement change management procedures prior to the installation of a new compressor; 5.) Failure to address the findings of their compliance audit.
