EPA filed a Consent Agreement and Final Order (CA/FO) in the matter of the University of Arizona Health Network, resolving Resource Conservation and Recovery Act (RCRA) violations at the Arizona Cancer Center in Tucson, Arizona. Violations alleged against the Respondent included: failure to mark and label containers of hazardous waste; storage of hazardous waste over 90 days; failure to properly train hazardous waste personnel or maintain training records; failure to properly maintain contingency plan; failure to maintain hazardous waste containers in good condition; failure to locate containers of reactive or ignitable waste at least 15 meters from facility fence-line; failure to properly maintain universal wastes; and failure to properly label universal waste. Respondent corrected the violations and agreed to pay a penalty of $15,300.
