A letter of voluntary self-disclosure under the Audit Policy was submitted to the US EPA on August 29, 2012. It was discovered that TRI Form R reports for formaldehyde should have been filed for calendar years 2007 through 2010. The required Form R reports were submitted on September 18, 2012. The report for formaldehyde for year 2011 was submitted on June 30, 2012, in a timely manner, as required.
