This is an administrative penalty action pursuant to Section 112(r)(7) of the CAA issued to Hawaii American Water for violations occuring at its East Honolulu Waste Water Treatment Plant in East Honolulu, HI. The violations involved failure to establish a system to track the status of action items identified during the Process Hazard Analysis process are addressed, and certify annually that operating procedures are current and accurate and that procedures have been reviewed as often as necessary, both as required under Section 112(r)(7) of the CAA. The Complaint / Consent Agreement establishes that Respondent will pay a penalty of $2,160.
