This was a Self Disclosure case from 2010. The respondent Belfor Environmetnal reported under EPA's Audit policy about RCRA and TSCA related violations. Regarding the PCB violations, an employee of Belfor improperly manifested and decontaminated PCBs in order to pocket the savings for himslef. Belfor discovered the incident and as a result the employee committed suicide. The facility reported to the violations under EPA's Audit Policy and the penalty was 100% mitigated.
