Violation Description: Failure to timely conduct mechanical integrity test. EPA sent an NOV identifying the above
violation. Operator promptly responded by performing a mechanical integrity test on the well. The well passed the
test. This violation was addressed with penalty orders consistently across the regulated community as part of an
effort to ensure operators are testing mechanical integrity in a timely manner.
