Respondent failed to monitor the System 's water for total coliform bacteria. After the System's water tested positive for total coliform on August 6, 2012, Respondent failed to take at least five routine samples of the System's water in
September 2012. Respondent failed to notify the public of the violations and Respondent did not notify EPA of the August 2012 MCL violation.
