Fail to complete CCR for 2011 and 2012. Did not receive the CCRs and Mr. Gibbons notified me in September that he no longer worked for Stateline Agri, Inc. and that the company was going out of business. OEPA confirmed that Stateline Agri, Inc. no longer owner of the PWS identified as OH1901012 as of late 2013.
