This enforcement matter related to 3 separate Clean Water Section 404 violations at three different sites within the State of Ohio (State Route 39 in Columbiana County, State Route 7 in Jefferson County and State Route 124 in Jackson County). The Administrative Order required a combination of on site restoration and mitigation (both on and off site) at each of the sites.
