EPA conducted an inspection of the Facility on May 1-4, 2018, to determine compliance with Section 112(r) of the CAA, 40 CFR Part 68. EPA determined there were 9 violations of Part 68 for Failure to Design and Maintain a Safe Facility, Operating Procedures, Failure to Implement Safe Work Practices, Failure to Conduct Mechanical Integrity Inspections, Failure to Implement Written Procedures to Maintain the On-Going Integrity of Process Equipment, Emergency Response Plan, and Failure to Timely Update/Correct the RMP.
