On 05/20/2022 Region 5 filed a Field Citation and Settlement Agreement against FAA Cleveland PVY ALS. In it, Region 5 alleges that FAA Cleveland PVY ALS failed to properly meet the requirements for periodic testing and monitoring of spill prevention equipment; failed to properly and periodically inspect overfill prevention equipment; failed to maintain any record of any calibration, maintenance and repair of release detection equipment; and failed to properly to maintain any documentation of compliance with release detection requirements. FAA Cleveland PVY ALS will pay a fine of $2,480.
