Follow up identified a new Dental Practitioner, Dr. Jon McDonald DDS, under a different Practice name, Los Alamos Dental and location, 185 Crystal Park Square. Practice agreed to accept and submit the OTCR as required by the Order. OTCR AO emailed 9/19/22. 9/19/22 Email address bounced. 9/21/22 F/U Phn call to update contact information. Spoke with Lindsey Hamilton, Receptionist. Dr. Moss no longer with the practice and it has relocated with a new name. Current information is Dr. Jon McDonald, Los Alamos Dental, at 185 Central Park Square. Lindsey agreed to receive the original AO with OTCR.
