The facility failed to perform the following: maintain documentation of compliance with release detection requirements; periodic testing of containment sumps and spill prevention equipment; inspect overfill prevention equipment; conduct periodic walkthrough inspections; annually test components of release detection; designate each individual who meets definition of Class C operator; and maintain a list of designated operators.
