This Administrative Order is issued to the Church of Jesus Christ of Latter Day Saints Frenchtown for failure to monitor for bacteriological quality; failure to report bacteriological monitoring results to the State within the required time frame; failure to provide public notice of the violations; and failure to notify the State of the violations.
