of the Notice of Intent confirmation letter, and a brief description of the project including the permit number. 2. Failure to develop an adequate Storm Water Pollution Prevention Plan.
3. Failure to install adequate control measures and stabilization practices at the construction site.
4. Failure to perform required inspections of the construction site.
