Respondent operated a hazardous waste facility without a permit by failing to maintain conditions exempting the facility from necessity of a permit; such a failing to implement a hazardous waste program training program; failure to maintain an adequate contingency plan; failure to label hazardous waste container and storing hazardous waste for longer than 90 days. No penalty was levided due to the demonstrated inability pay of the respondent.
