As a result of EPA's January 23, 2002 inspection, EPA determined that Medrad; failed to comply with initial notification requirements of 63.366(c); conducted late and flawed performance test in violation of 63.363, without notice to EPA required by 63.9(e); failed to monitor & record required parameters under 63.364(b); and failed to submit semi-annual reports under 63.366(a)(3) of the Ethylene Oxide NESHAP at its facility in Indianola, PA.
