'Violations: 1) Certified DMS filed by Permittee show discharges of pollutants from the facility that exceeded permitted effluent limitations; 2) State Inspection unsatisfactory; and 3) permit application not received.
**Relief: 1) Within 30 days, the Permittee shall take action to correct violations; 2) within 30 days, Permittee shall submit written report of actions taken; 3) if correction of violations is not possible within 30 days, Permittee shall submit comprehensive plan to do so; and 4) the Permittee should arrange a Show Cause meeting within EPA in 45 days.'
