'**VIOLATION: 1) Discharging pollutants from a point source to waters of the U.S. without a permit. **RELIEF SOUGHT: A) Immediately take measures to prevent any discharge of pollutants to waters of the U.S. and demonstrate that the facility is properly designed, consytructed, operated , and maintained; B) Within 30 days, initiate a weekly visual inspection program to determine if any discharges are occurring and notify EPA of any discharges from the facility by phones within 24 hours and by writing in five days; C) Within 30 days, submit information concerning any discharges of pollutants from the facility to waters of the U.S. within the last five years; and D) submit a Notice of Intent (NOI) to be covered under the EPA CAFO general permit.
