'**VIOLATIONS: 1) Failing to prepare and implement an operations and maintenance plan; 2) Failing to conduct a performance test to demostrate compliance for the hard chromium electroplating tanks; 3) Failing to monitor each affectedsource to demonstrate ongoing compliance; 4) Failing to submit an initial notification of compliance status upon completion of the performance test; and 5) Failing to begin preparing an annual on-going compliance status report, to be retained on site and made available upon request. **RELIEF SOUGHT: a) prepare and implement an operations and maintenance plan within 45 days; b) Conduct a performance test to demonstrate compliance for the hard chromium electroplating tanks within 60 days; c) monitor each affected source to demonstrate ongoing compliance within 60 days; d) Within 90 days, submit an initial notification of compliance status upon completion of the performance; and e) Within 180 days, begin preparing an annual on-going compliance status report, to be retained on site and made available upon request.
