Respondent failed to submit a completed emergency and hazardous chemical inventory form for the listed chemicals to the SERC for calendar years 1997, 1998 and 1999, and to the LEPC and fire department with jurisdiction over the facility for calendar year 1997, by March 1 of of the year followinf the calendar year for which the report was required, and Respondent failed to include silver in the completed emergency and hazardous chemical inventory form submitted to the LEPC and the fire department with jurisdiction over the facility, for calendar year 1998.
