These failures include: (1) monitoring and reporting requirements for total coliform bacteria, nitrate, and nitrite; (2) notification to the state of NC of all SDWA violations; and (3) notification to customers of violations of the SDWA. On March 20, 2002, EPA issued an Administrative Order, Docket No. SDWA-04-2002-0754. Respondent failed to comply.
