Respondent operates injection wells in Osage County, Oklahomas as provided by mineral mining leases with the Osage Nation. Respondent is subject to regulations under the Safe Drinking Water Act, Underground Injection Control (UIC) Program. Applicable UIC regulations require that Respondent submit an annual report of well operations. The report must include average and maximum monthly injection pressure and volume of fluids injected each month. Respondent failed to submit the required reports for 10 injection wells for the period January through December 2001. The AO requires Respondent to submit the reuired report.
