AO issued 12/24/02. VIOLATION: Respondent operated the construction site from 10/28/00 to 04/06/01 without permit coverage. After permit was effective, required documentation was not posted at the entrance, there were no control measures, and the entrance to the facility was not properly stabilized.
**RELIEF SOUGHT: 1) Within thirty days, Respondent shall take whatever corrective action is necessary to correct the deficiencies, eliminate and prevent recurrence of the violations, and submit a written report detailing the specific actions taken; 2) If complete correction of the violations is not possible within thirty days, submit a comprehensive plan to do so; and 3: Within forty five days, arrange a Show Cause meeting with EPA
