**RELIEF SOUGHT: 1) Within thirty days, the Permittee shall take whatever corrective action necessary to eliminate and prevent recurrence of the violations and shall submit a written report detailing the specific actions taken; and 2) If complete correction of the violations is not possible within thirty days, submit a comprehensive plan to do so;
