VIOLATION: Respondent conducted activities and operated the facility without NPDES permit coverage.
** RELIEF SOUGHT: 1) Within thirty days, Respondent shall make a complete and correct application for permit coverage, and shall submit a copy of the permit application and confirmation of coverage to EPA; 2) Within thirty days, Respondent shall take whatever corrective action is necessary to correct the deficiencies and eliminate and prevent recurrence of the violations, and shall submit a written report detailing the specific actions taken; 3) If complete corrections of the violations is not possible within thirty day, submit a comprehensive written plan to do so; 4) Within thirty days, Respondent shall submit a list of all construction sites begun and/or completed since June 1, 2001; and 5) Within forty-five days, arrange a Show cause meeting with EPA.
