Violations: 1. The required notice was not posted at the entrance of the construction. 2. The construction site's storm water pollution prevention plan (SWPPP) was not properly signed and certified. 3. Storm water inspections were not being conducted in accordance with Part IV.D.4. of the SWGCP. 4. The entry/exit construction entrance was in need of maintenance to prevent further offsite tracking of mud and sediment.
