Respondent faield to comply with final limitations and controls specified for storm water discharged from the site, as evidenced by the discharge of sediments from the site. Respondent failed to properly operate and maintain facilities and systems of treatment and control to achieve compliance with conditions of the Permit, as evidenced by the discharge of sediments from the site and the lack of maintenance around the site. Repondent failed to comply with Parts I.A.2., I.B.2., I.B.3, I.C.1., and I.C.2., and II.C.1. of the NCDENR Permit and also for discharges not authorized by the NCDENR Permit
